First Visit
Welcome to Forest Hills Family Dental. Our team is excited to meet you! At FHFD, we truly provide a great dental experience for our patients—one that helps you achieve your dream smile with affordable treatments tailored to your needs. With a goal to help you achieve and maintain great oral health, we offer a comprehensive range of services and treatments. Our dentists, Dr. Stuart Pettijohn & Dr Jarred Ewert, work together each day to provide convenient dental care that exceeds your expectations. Office Tour
We’ll show you around our office so you can become familiar with where the exam rooms, bathroom & exits are located. However, we try our best to provide attentive care so you’ll rarely be left alone during your appointment unless necessary.
Digital X-Rays
An important part of your first visit are x-rays. These provide an in-depth view of your oral structures & the high resolution images make it possible for our dentists to find cavities, small cracks, & other problems with your teeth.
Examination
One of our dentists will complete a thorough oral examination of your tissues, gums, bone & teeth. We’ll check for decay, disease & infection, and complete an oral cancer screening. This is also the perfect time to discuss what you do & do not like about your smile, as well as any dental problems that you may be experiencing.
Hygiene Cleaning
Finally, we’ll complete a dental hygiene cleaning. Our hygienist will remove plaque & tarter buildup from your teeth, carefully floss each tooth, & remove any stain on your teeth by polishing.
Take Home Goodies
You will leave our office with a gift bag of items, including floss (or other aides), toothpaste, and a new toothbrush. Now you can enjoy your newly cleaned teeth & maintain proper oral health until your next visit!

Our high-tech intraoral scanner takes precision scans of your teeth & gums – no more bad tastes or gagging during impressions. We use these scans for planning restorative treatments.
CEREC Same-Day Crowns
Restore your damaged tooth in just one appointment with CEREC same-day restorations. We can scan your tooth, design a new dental crown, fabricate it in-office in our milling machine, and attach the restoration – all within two hours.
Isodry Isolation System
Isodry is the only dental device that provides suction and retraction in one easy-to-use tool. For the patient, it makes for faster, safer, & more comfortable procedures!
With Isodry, patients don’t have to strain to keep their mouth open the entire procedure. The built-in bite block allows the patient to comfortably rest their jaw while keeping their mouth open at all times.
The soft, flexible mouthpiece also provides isolation with retraction & continuous suction of fluids. This combination translates to comfortable & easy moisture control; it eliminates the choking sensation caused by water pooling in the back of the mouth.
Isodry also serves as a barrier; allowing for throat, tongue & cheek protection. The tongue is completely out of the way – no more worrying about placement throughout the procedure! Insurance & Finance
Thank you for choosing Forest Hills Family Dental as your dental health care provider. We believe that all patients deserve the very best dental care that we can provide. Our providers will not allow your dental insurance to dictate your oral health care. Please don’t let financial concerns stop you from getting the oral health care your smile deserves. We encourage you to contact our office today to learn about your insurance and financing options. We also believe that everyone benefits when specific financial arrangements are agreed upon. Please understand that payment of your bill is considered a part of your treatment.

Full payment and/or co-payment and deductibles are due at the time of service.
We accept cash, checks, VISA, Mastercard, Discover & American Express credit cards & debit cards.
Dental Insurance
As a courtesy, we will bill your dental insurance provider directly. Please keep in mind, however, that your insurance policy is a contract between you and your insurance company.
We cannot properly bill your insurance unless you present to us all of your correct insurance information at your initial visit. Likewise, please remember to let us know at the beginning of each visit if there are any updates or changes to your insurance policy and/or your employment status so that we can assist you in maximizing your benefits.
It is the patient’s responsibility to know, understand & track their insurance benefits, deductibles & maximums. Most policies do not cover 100% of the cost of your treatment. Because of this, and the delay in receiving payment from the insurance company, the patient will be responsible to pay the deductible, co-payment, and/or the portion of the charges on the day the services are rendered. Also, since we are not an involved party with your insurance contract, you are responsible for any non-payment on our account.
We will estimate your coverage, but until we actually receive the payment from the insurance company, it is just an estimate. We also encourage you to contact your insurance company if you have questions about whether a service is a covered benefit, keeping in mind that they will never guarantee payment. We will assist you in dealing with the insurance company, but ultimately the responsibility lies with you.
We normally wait 45 days for your insurance company to provide payment for services. If after the 45-day period there is still non-payment, the balance on the account will be billed to you.
Third-Party Financing
We partner with CareCredit® to offer additional financing for elective treatments and procedures not covered by dental insurance, such as dental implants. These plans provide low monthly payments with competitive interest rates.

Cancellation / Failed Appointment Policy
Our goal at Forest Hills Family Dental is to provide quality dental care in a timely manner. We do understand that illness, emergencies, and bad weather do occur. We ask our patients to give us 24 hours notice whenever possible, if you cannot keep an appointment. This allows us time to fill our schedule with other patients who may be waiting. We appreciate your understanding and consideration regarding our cancellation and failed appointment policy.

- Cancellation or rescheduling of an appointment with more than 24 hours or more notification will result in no charge.
- A failed appointment is an appointment that is cancelled/rescheduled without 24 hours’ notice or an appointment where a patient does not show up. · We do allow for one (1) broken appointment as a courtesy.
- Any additional failed appointments will be charged a fee of $50.
- Excessive cancellations and no shows will result in termination of our treatment agreement and your records can be forwarded to another dental office for a fee of $10.
